Lasix (Furosemide) is a medication routinely used to prevent kidney disease in patients with heart failure. However, its use in critically ill patients is only rarely investigated. This study aimed to assess the efficacy and safety of furosemide in the treatment of acute kidney injury (AINH) in critically ill patients.

During the period 2017 to 2022, an intravenous infusion of furosemide in critically ill patients was evaluated and compared with intravenous furosemide in three different studies. In these studies, patients with AINH (mean creatinine clearance [ creatinine clearance (CLcr] [2.6 mg] and mean creatinine urine [mg/mL] were included) and those with normal renal function were included. In the two studies that included a subset of patients with AINH, furosemide was superior to intravenous furosemide in terms of clinical efficacy and renal clearance ( [Table 1 and ] ). The differences in the median creatinine clearance (CLcr) between both studies were not statistically significant (P<0.05). In the subgroup analysis of patients with AINH with CLcr <80 mL/min/1.73m2 and in the subgroup analysis of patients with CLcr ≥80 mL/min/1.73m2, the median renal function (RFR) was significantly lower in the furosemide group compared with the intravenous group (<0.01). There were no differences in the RFR between the two groups (=0.86). The median duration of treatment was significantly longer in the furosemide group than in the intravenous group (=0.04; [Table 1 and ] ).

Table 1Clinical efficacy of furosemide and intravenous furosemide in the treatment of acute kidney injury

The three studies evaluated the efficacy and safety of furosemide for acute kidney injury, including the treatment of acute interstitial nephritis (AINH). Furosemide is a potent loop diuretic (also known as loop diuretics) which has been approved in several European countries for the treatment of AINH. Furosemide was shown to improve renal function and reduce the incidence of acute interstitial nephritis in AINH patients with normal renal function. The most common adverse events were headache, nausea, vomiting, and abdominal pain, although most of these were mild. In addition, furosemide was associated with increased risk of death, including both direct renal complications and hospitalization for renal impairment. However, furosemide was not associated with the risk of major adverse cardiovascular events in AINH patients. In addition, furosemide was not associated with renal papillary necrosis or acute renal ischemia in AINH patients. The most common adverse events were headache, nausea, vomiting, and abdominal pain. However, the most common adverse events in AINH patients with normal renal function were headache (n=26), nausea, vomiting (=3), and abdominal pain (=2). In a large retrospective cohort study involving a total of 13,835 critically ill patients with acute interstitial nephritis, the most common adverse events were headache (=10), nausea, vomiting (=6), and abdominal pain (=6).

In this retrospective cohort study, the most common adverse events occurred in patients with normal renal function (≥80 mL/min/1.73m2) and in those with an RFR <80 mL/min/1.73m2 (≥8). Headache (=2), nausea (=2), vomiting (=2), and abdominal pain (=1) were the most common adverse events in the furosemide group, and the most common adverse events were headache (=3) and nausea (=3).

In the subgroup analysis of patients with normal renal function, the most common adverse events were headache (=8), nausea (=7), abdominal pain (=7), and diarrhea (=7).

Lasix 40mg tablet contains an active ingredient called Furosemide which belongs to the medication class known as diuretics. It is used to treat edema (excessive water accumulation) and high blood pressure. Edema can be caused by various underlying issues such as heart, lung, kidney, or liver problems. This helps the kidneys to remove excess water, which is not needed by the body.

Lasix 40mg tablet should not be taken if you are allergic to Furosemide or other sulphonamide-related medicines or any of the ingredients in the medication. Also, do not consume if you have anuria, impaired kidney function or kidney failure, severe kidney damage, very low levels of potassium, sodium, or other electrolytes, dehydration, low blood pressure, take potassium supplements or potassium-sparing diuretics, have liver cirrhosis or liver encephalopathy, suffer from Addison's disease, have digitalis poisoning, or if you are breastfeeding.

Before starting the treatment with Lasix 40mg tablet, notify your doctor if you have hypovolaemia (low blood volume) or risk of low blood pressure, hypoproteinaemia (low levels of blood protein) due to kidney damage, liver congestion or other liver problems, kidney problems, diabetes or insulin use, advanced age or medications that lower blood pressure, prostate issues or difficulty in urinating, history of gout or abnormal blood condition, and upcoming blood or urine tests.

You should have anuria.

To prevent the development of anuria, further medical tests should be taken including bone scanning and/or urine tests, as these may be the first signs of anuric symptoms, before the treatment with the medication is completed. In case you have not been urothelialised in which you are suspected to have prostate cancer, DICP (dissolving in a liquid) may be required to take into account the impact of dosing on symptoms, as well as the impact of the medication on the patient's health. You should especially report to your doctor any symptoms such as 'hoardie', 'hairyhead', 'puffy' or'stiff' chest, low back pain, easy walking, feeling weak or 'uncommitted', difficulty with breathing, nausea, vomiting, or fainting as this may be the indication of anuria or other complications of the disease. You should also avoid consuming medications that contain sulphonamides (e.g., sulphonamide-containing medicines, ketoconazole medicines, and quinapril) as they can affect your kidney function and lead to anuric symptoms (yeast pain or sore throat).
